This form intends to communicate what may be needed/reviewed to assist with the planning and preparations for an upcoming Club Sports event taking place on the UW-Whitewater campus.

Please submit 48-72 hours prior to your event at the very latest, as this will give Club Sports administrative staff enough time to assist with gathering/preparing items/facilities/staff.
