Club Sports Dues Acknowledgement & Agreement

Dues Acknowledgement & Agreement Form for the UW-Whitewater Club Sports program. It is strongly encouraged that this form is on-file each academic year in which you participate with a Club Sports team/club; especially if a club collects membership dues.

Club Sports Dues Acknowledgement & Agreement

Before completing this form, we ask you understand the following:

PAYING OR NOT PAYING DUES DOES NOT IMPACT AN INDIVIDUAL'S ELIGIBILITY TO ATTEND AND PARTICIPATE IN ON-CAMPUS ACTIVITY. ALL MEMBERS OF THE CAMPUS COMMUNITY ARE WELCOME TO PARTICIPATE IN CLUB SPORTS


Each individual club recognized by the Office of Recreation Sports & Facilities has the option to collect member dues. The intent to collect dues should be clearly stated by clubs in their constitution and/or budget


Clubs may also come to any type of agreement with its general membership regarding how dues are collected (i.e. frequency, leniency, payment plans, etc.)


All members participating within a club who has elected to collect dues should complete the Dues Information & Agreement. Each member should sign this form prior to being active with the club in all activities so all parties are on the same page regarding financial obligations and expectations. If a club charges member dues and a member has not signed this agreement, they may be subject to receiving ineligible status for off-campus activity, which means members may not be eligible to be on an applicable governing body roster, travel, compete, receive reimbursement and/or receive equipment from the specific club mentioned below


If a member has not signed this agreement or has not made their dues intentions clearly known and is found to have been participating with the club mentioned below in a specific capacity, individual club leadership will be held accountable and further action may be taken at the discretion of administrative staff. If a member has signed this agreement and has not fulfilled their obligation to pay dues or notify club leadership of their intent to pay dues within a reasonable timeline, club leadership, club advisors and the Office of Recreation Sports & Facilities reserve the right to request a meeting with the individual in question to determine the individual's eligibility to receive club participation benefits


All decisions rendered by the Assistant Director of Club Sports are final, upon appeal to Whitewater Student Government


All dues paid via check should be written out to “UW-Whitewater [Club Name].” Dues should be collected by club leaders and submitted to the Assistant Director for proper processing/advisement
